Consequence
BOLT HEAD IMPINGMENT CAN CAUSE THE PEDESTAL BOLT RETNETION SYSTEM TO LOSE THE SPECIFIED CLAMP LOAD CAUSING THE CONCRETE DRUM TO SEPARATE FROM THE TRUCK CHASSIS, INCREASING THE RISK OF SERIOUS INJURY OR DEATH.
Remedy
OWNERS HAVE BEEN ADVISED TO IMMEDIATELY CHECK ALL BOLTS ON THE REAR PEDESTALS. IF ANY ONE HORIZONTAL LOCKBOLT IS LOOSE OR MISSING, TO IMMEDIATELY REPLACE THE LOOSE LOCKBOLT.
